Men typically have 3 – 5 erections per 8 hours of sleep, usually at night. This is called nocturnal penile tumescence (NPT). The NPT test shows whether these erections have occurred.
Penile Doppler ultrasound is an important diagnostic tool used to see how blood flows in and out of the penis during and after an erection. This specialized imaging study looks at the blood flowing into the penis through the arteries (causing an erection) and flowing out of the penis through the veins (returning to flaccid) as well as surrounding nerves and tissue structures to identify any scarring or calcification. .

The recommended starting dose of tadalafil for use as needed for most patients is 10 mg taken orally approximately one hour before sexual activity. A doctor may adjust the dose higher to 20 mg or lower to 5 mg depending on efficacy and side effects. Doctors recommended that patients take tadalafil no more frequently than once per day. Some patients can take tadalafil less frequently since the improvement in erectile function may last 36 hours. Patients may take tadalafil with or without food. Tadalafil is currently the only PDE5 inhibitor that is FDA-approved for daily use for erectile dysfunction and is available in 2.5 mg or 5 mg dosages for daily use.
In 2018, the Journal of performance Medicine published a review of 42 studies on ED and depression. Together, the studies included over 192,000 men. The authors reported that men with depression had a 39% increased risk for ED. And men with ED were almost three times more likely to have depression than men who had no trouble with erections.

However, sildenafil should be used carefully with nitrates because their combination can result in severe hypotension and death.68 Both short- and long-acting nitrates are commonly prescribed to treat angina, but they have no prognostic benefit. In addition, there are numerous alternatives to treat angina, such as ranolazine and ivabradine, which do not interact with PDE5 inhibitors. As a result, patients with ED wishing to take PDE5 inhibitors can safely discontinue their nitrates and replace this treatment with the other anti-anginal agents.68
