Second-generation cardioselective beta-blockers (atenolol, metoprolol, bisoprolol, etc.) can also lead to ED. Atenolol was shown to cause significant reduction of sexual activity compared with placebo in a double-blind, parallel-arm study.22 The same study also showed a significant reduction in testosterone levels with atenolol versus valsartan. An open, prospective study of hypertensive men treated with atenolol, metoprolol and bisoprolol for at least 6 months showed high prevalence of ED – approaching 66 % – in these patients.23

Kegel exercises, also called pelvic muscle exercises, are performed to strengthen the muscles of the pelvic floor, which can help prevent incontinence, prevent accidentally passing gas or stool, and improve orgasms.

Age is a critical risk factor for the development of ED and endothelial dysfunction.4,5 ED is the most common condition occurring in middle-aged and older men.5 Kinsey et al. reported that 25 % of 65-year-old men and 75 % of ≥80-year-old men have ED.39 Moreover, ageing also decreases endothelial function, which is responsible for IHD.5 The incidence and severity of ED increases with age (a man aged 70 years is three-times more likely to have ED than a man aged 40 years).40

Most men will experience occasional ED. This can be due to triggers such as stress, anxiety or drinking too much alcohol. Frequent erectile dysfunction can be a sign of an underlying health condition or emotional problems. If it happens regularly you should see your GP or visit a sexual health clinic.

Penile Doppler ultrasound is an important diagnostic tool used to see how blood flows in and out of the penis during and after an erection. This specialized imaging study looks at the blood flowing into the penis through the arteries (causing an erection) and flowing out of the penis through the veins (returning to flaccid) as well as surrounding nerves and tissue structures to identify any scarring or calcification.
All treatments are medically proven to assist in getting and maintaining an erection – but each one of them are unique in their abilities and effectiveness.
The diagnosis of erectile dysfunction is made clinically, based on a patient’s symptoms. It therefore is important for patients to be evaluated by their GP in the first instance. At this appointment, a full history will be taken from the patient regarding the current symptoms and other medical issues. Regular medications will also be reviewed to ensure that there are no contributing factors. Questions regarding alcohol intake and smoking will also be asked. A general physical examination will be performed by the GP, which may include checking blood pressure and an examination of the genitals. Examination of the vascular system and peripheral nervous system may also be undertaken.

When the aforementioned strategies prove insufficient to address ED symptoms, medical therapies are warranted. The simplest treatments include use of a class of oral medications known as phosphodiesterase type 5 inhibitors (PDE5I, e.g. sildenafil, tadalafil, vardenafil, and avanafil). These drugs enhance the effects of nitric oxide, a natural molecule within the body that relaxes the smooth muscle of the penis, to enable increased blood to flow and result in erection. These medications are generally very safe and reliable. Men who use nitrates (found in certain heart medications and in some recreational drugs) should not use PDE5I. Optimal use of PDE5I requires advice on timing and proper use of these drugs for maximal effect.
An enlarged prostate, or benign prostatic hyperplasia, is a common condition affecting men over 50. While uncomfortable, BPH is not life threatening.
Collagenases are proteinases that hydrolyze collagen in its native triple helical conformation under physiological conditions, resulting in lysis of collagen deposits. The signs and symptoms of Peyronie’s disease have been found to be caused by a collagen plaque. Injection of collagenase clostridium histolyticum into a Peyronie’s plaque, which is comprised mostly of collagen, may result in enzymatic disruption of the plaque. Following this disruption of the plaque, penile curvature deformity are reduced (Endo Pharmaceuticals, 2021). Measurement of Serum Melatonin Levels for the Diagnosis of Erectile Dysfunction
When a man has problems with erections, it is sometimes difficult to know if the cause(s) are physical, psychological, or both. The nocturnal penile tumescence (NPT) test can give the doctor some clues.
Exercise that results in losing weight, particularly around the waistline, can cut a man’s risk of ED. A man with a 32-inch waistline has only half the likelihood of erectile dysfunction as a man with a 42-inch waistline. Losing excess weight is one of many factors that can affect penile dysfunction.
