ED can be stressful for men and their partners, and depression can develop from relationship conflict. The man with ED might worry that he is not meeting his partner’s needs and feel that he is inadequate or less masculine. His partner might worry about being attractive enough or suspect that the man is having an affair. If these feelings aren’t brought out into the open, the situation can worsen, with both partners withdrawing, avoiding each other, or missing the intimacy that they once shared.
Unlike other treatments for erectile dysfunction, PDE5i medications requires sexual stimulation to function. Without stimulation, these medications will not provide any effect.
However, sildenafil should be used carefully with nitrates because their combination can result in severe hypotension and death.68 Both short- and long-acting nitrates are commonly prescribed to treat angina, but they have no prognostic benefit. In addition, there are numerous alternatives to treat angina, such as ranolazine and ivabradine, which do not interact with PDE5 inhibitors. As a result, patients with ED wishing to take PDE5 inhibitors can safely discontinue their nitrates and replace this treatment with the other anti-anginal agents.68
Other treatments also have possible side effects and complications. Ask your provider to explain the risks and benefits of each treatment.
The recommended dosage of sildenafil is 50 mg/day, usually taken 1 hour before sexual activity. This dose may be increased to 100 mg or decreased to 25 mg based on side effects.6 PDE5 inhibitors also have a beneficial effect in the treatment of heart failure with reduced ejection fraction as well as pre- and post-capillary pulmonary hypertension. The use of PDE5 inhibitors in the treatment of right heart failure and left ventricular failure associated with combined pre- and post-capillary pulmonary hypertension has been well studied.71,72
Impotence does not go hand in hand with aging. Other factors, such as a person's physical health and emotional well-being, are more likely to be the reason for the lack of sexual performance in an individual.

Picture of inflatable penile prosthesis. This inflatable penile prosthesis has fluid located at the base of the device. When the tip of the device is squeezed, the fluid is transferred into the cylinder. Erection problems usually produce a significant psychological and emotional reaction in most men. This is often described as a pattern of anxiety, low self-esteem, and stress that can further interfere with normal sexual performance. This "performance anxiety" needs to be recognized and addressed by your health care provider.
Many factors can contribute to sexual dysfunction in older men, including physical and psychological conditions, comorbidities and the medications used to treat them. Aspects of an ageing man’s lifestyle and behaviour and androgen deficiency, most often decreasing testosterone levels, may affect sexual function as well. A study of men between the ages of 30 and 79 years showed that 24% had testosterone levels below 300 ng/dL and 5.6% had symptomatic androgen deficiency.2
If you have erectile dysfunction, there are many ways your doctor can help you treat it. But you may want to consider complementary or alternative therapies, such as nutritional supplements, herbal remedies, acupuncture, and talk therapy. Just be sure to talk to your doctor first.
Despite conflicting findings in clinical studies, men should monitor the effects of alcohol use on erectile function. Alcohol can lower testosterone levels, decrease blood flow into the penis, and cause fatigue. At least in some men, alcohol can exacerbate symptoms of E.D.
